Once an application for a temporary fireworks stand has been received and approval given upon condition of approval for a site plan, the applicant must obtain two inspections and a DRC temporary site plan packet must be completed before a temporary business license can be issued.
A business license inspection must occur which the applicant will call (801) 229-7060 to schedule.

It is the applicant's responsibility to ensure that both inspections have been scheduled and a DRC temporary site plan packet submitted. Both inspections and DRC temporary site plan packet will need to occur and pass prior before a business license can be issued. Applicant must not operate the business without a business license. Should the applicant commence business prior to a business license being issued, the applicant will be subject to a citation written by a police officer.
By signing this form, the applicant is acknowledging that they understand that a citation for working without a business license may occur if both inspections have not been completed by the applicant.
If I conduct business without a license, I may be subject to penalties as described above.
